Terms of Service
By using Live Well Psychiatry website, forms, and submitting information electronically, you acknowledge and agree to the following terms.
Provider-Patient Relationship & Prescribing
Live Well Psychiatry provides outpatient mental health services, including telehealth-based psychiatric care and related administrative support.
- Prospective Patients: Submitting a form or inquiry does not establish a provider-patient relationship unless and until you have been formally accepted, scheduled, and clinically evaluated by a Live Well Psychiatry provider.
- Established Patients: Submitting an electronic request does not guarantee a specific clinical response, medication refill, prescription change, medical letter, form completion, treatment recommendation, or appointment outcome.
- All clinical and medication requests are subject to review and professional medical discretion by our clinical staff and providers.
Emergency & Crisis Notice
Our website, electronic forms, and email channels are not intended for urgent or emergency situations and are not monitored 24/7. Do not use these forms to report a medical emergency, mental health crisis, suicidal thoughts, homicidal thoughts, medication emergency, or urgent safety concern.
If you are in immediate distress or danger:
Call 911, call or text 988 (Suicide & Crisis Lifeline), go to the nearest emergency room, or contact local crisis services immediately. For more details, see our Emergencies Page.
Telehealth Services & Limitations
Telehealth psychiatric care involves remote technology and has specific parameters:
- Telehealth services have inherent limitations, including potential technology issues, connection disruptions, privacy factors in your physical environment, and the provider’s inability to conduct in-person physical assessments.
- You agree to participate in telehealth visits from a private, safe, and appropriate location within the State of Idaho.
- For safety and regulatory compliance, you may be asked at each visit to confirm your current physical location, phone number, emergency contact, and nearby emergency facilities.
Accuracy of Information & Communications
You agree that all information you provide to Live Well Psychiatry is accurate and complete to the best of your knowledge. False, incomplete, outdated, or misleading information may delay services, impact treatment decisions, affect medication management, or result in discharge from care.
Electronic communication—including online forms, text messaging, email, and patient portal messages—carries inherent security risks despite industry safeguards. You acknowledge these risks and agree to use our communication tools only for appropriate non-emergency communication.
Clinic Policies, Refills & Conduct
You agree to adhere to all Live Well Psychiatry office policies, including attendance, cancellations, medication management, controlled substance agreements, prescription refill protocols, billing, insurance, and appropriate conduct. Failure to follow clinic policies may result in treatment delays, service limitations, or dismissal from the practice when clinically and legally appropriate.
For office policies and billing details, visit our Patient Forms and Insurance FAQs.
Updates & Contact Information
Live Well Psychiatry reserves the right to update these terms as needed. Continued use of Live Well Psychiatry forms, website, and electronic communication methods constitutes acceptance of the current terms.
